Consumer Health Data Privacy Policy
Last updated: September 22, 2026
This Consumer Health Data Privacy Policy explains how Alliance Recovery Center LLC (“Alliance Recovery Center,” “we,” “our,” or “us”) collects, uses, and shares “consumer health data” as defined by Washington’s My Health My Data Act (RCW 19.373). It applies to Washington residents whose consumer health data we handle through this website. In practice, we extend the protections described here to all visitors.
This policy supplements — it does not replace — our Privacy Policy and our Notice of Privacy Practices.
1. Categories of Consumer Health Data We Collect
We collect the following categories of consumer health data, which is information that identifies your past, present, or future physical or mental health:
- Identifiers you provide: name, phone number, email address, and mailing address when you contact us or request an appointment;
- Health-related information you share: the program or service you are asking about (for example, mental health treatment, substance use treatment, or our day program), insurance information, preferred contact times, and anything you tell us about your health concerns or treatment needs in forms, emails, or calls;
- Automatically collected information: device and browsing information (such as IP address, browser type, and pages visited) that may reflect an interest in health services.
Please keep messages brief and do not include Social Security numbers, detailed diagnoses, or other highly sensitive details in website forms or email.
2. Purposes for Which We Collect and Use Consumer Health Data
We collect and use consumer health data only for the following purposes:
- To respond to your appointment requests, questions, and messages;
- To verify insurance coverage and schedule or coordinate care at your request;
- To operate, maintain, and secure this website; and
- To comply with applicable laws and legal processes.
We do not use consumer health data for any other purpose without first obtaining your affirmative consent.
3. Sources of Consumer Health Data
We collect consumer health data directly from you — through our appointment request form, phone calls, and emails — and automatically through your use of this website as described above.
4. Categories of Third Parties and Processors With Whom We Share Consumer Health Data
We share consumer health data only with the following categories of recipients:
- Service providers (processors) that help us operate the website, such as our website hosting provider and the provider that processes our online forms. They may use the data only to provide services to us.
- Legal authorities when required by law, court order, or legal process.
We do not sell consumer health data. We do not share consumer health data with third parties except as described above and with your affirmative consent where the law requires it.
5. Your Rights Over Your Consumer Health Data
Under Washington’s My Health My Data Act, you have the right to:
- Confirm whether we are collecting, sharing, or selling your consumer health data;
- Access your consumer health data, including a list of the third parties and processors with whom we have shared it and an active contact for each;
- Receive a copy of your consumer health data in a portable format;
- Delete your consumer health data; and
- Withdraw consent you previously gave for our collection or sharing of your consumer health data.
How to exercise your rights. Call us at (206) 468-6359 or email info@alliancercnt.com with “Consumer Health Data Request” in the subject line. We will respond within 45 days. If we need more time, we may extend once by another 45 days and will tell you why. If we cannot verify your identity from the information you provide, we may ask for additional information; if we still cannot verify you, we may decline the request and explain why.
Appeals. If we deny your request, you may appeal by replying to our response or contacting us again within a reasonable time. We will review the appeal and respond in writing within 45 days, explaining our decision and how to contact the Washington State Attorney General if you disagree.
We will not discriminate against you for exercising any of these rights.
6. Consent
We obtain your affirmative consent before collecting consumer health data, and before sharing it, except as necessary to provide a product or service you requested or as otherwise permitted by law. You may withdraw your consent at any time using the contact methods above.
7. Geofencing
We do not use geofencing technology to identify, track, or collect information about visitors to health care facilities, and we do not send notifications related to health data based on such technology.
8. Data Retention
We retain consumer health data only as long as reasonably necessary to fulfill the purposes described in this policy, to maintain our records, and to meet legal obligations. When data is no longer needed, we delete it or de-identify it.
9. Changes to This Policy
We may update this policy from time to time. When we do, we will revise the “Last updated” date above and post the new version on this page. If changes materially affect how we handle your consumer health data, we will obtain your consent where required by law.
